Do Not Enter Protected Health Information

RehabAssistantTool is not intended for the storage of Protected Health Information (PHI) unless a healthcare organization has implemented appropriate HIPAA-aligned infrastructure and agreements.

What is PHI?

Protected Health Information (PHI) includes any individually identifiable health information that relates to a person's past, present, or future physical or mental health condition, provision of health care, or payment for health care.

Information Users Should NOT Enter

Do not enter any of the following into any text field within RehabAssistantTool:

  • Patient names or initials that could identify an individual
  • Dates of birth
  • Addresses (home, work, or other)
  • Phone numbers
  • Medical record numbers (MRNs)
  • Social Security Numbers
  • Insurance member numbers or IDs
  • Account numbers
  • Certificate or license numbers
  • Device identifiers
  • URLs or IP addresses linked to individuals
  • Any other unique identifying information

PHI Detection

RehabAssistantTool includes automated PHI detection warnings within text fields. These warnings are intended as a reminder tool only and do not guarantee the detection of all PHI. Users remain solely responsible for ensuring no PHI is entered into the platform.

Business Associate Agreement (BAA)

RehabAssistantTool does not provide a Business Associate Agreement (BAA) under its standard subscription. Healthcare organizations requiring a BAA for HIPAA compliance must contact us directly to discuss enterprise arrangements before using the platform with any PHI.

Your Responsibility

Each user and their employing organization is responsible for ensuring compliance with HIPAA and all applicable federal and state privacy laws when using RehabAssistantTool. The platform's built-in PHI warnings are a courtesy feature and do not constitute legal or compliance guidance.
